Network Connection Application Forms

Applications for upgrades to existing connections or requesting new connections to Horizon Power's networks.  The amount of information required and the time taken to process an application will depend on your supply requirements (based on your load).  Please note all connections are to be in accordance with Western Australian Electrical Requirements (WAER).

Application for Underground Supply in an Overhead Area

This form is used for Horizon Power to install a service line from the overhead network to your underground pillar (often referred to as pole to pillar).

Contributory Extension Scheme Inquiry Form

The Supply Extension Scheme operates in a number of regional areas supplied by Horizon Power.  If you are purchasing a property in a rural area, it may be that the property you are buying has an annual charge applicable under a Scheme which has been operating for a number of years.  To find out if this applies to you, please complete the online enquiry form below and further information will be provided to you.

  • CES Online Form

Design and Quotation Application (DQA)

The Design and Quotation Application (DQA) for an Electrical Network Connection must be used for applications where:

  • it is for a commercial or industrial property;
  • it is for more than three domestic dwellings; or
  • it has been deemed excessive or disturbing.

Due to the large number of variables in the required supply and the network, it is not possible to state which parts of the DQA are mandatory fields.  If there are any queries they should be directed to Horizon Power on telephone (08) 9441 3710 or the Design Project Manager if one has been nominated for your project.

Request for Horizon Power Network Service

This form can be used to arrange connection or disconnection of power to your property.

Request for Quotation for Relocation of Horizon Power Assets

This request form is to be used to obtain a quotation to move Horizon Power assets (such as a supply pillar or network pole in the street).  Relocation of an asset will be subject to a suitable alternate location.  All costs involved in moving assets will be chargeable to the requestor.

Supply Extension Scheme (SES) Application Form

Horizon Power extends its overhead/underground distribution network to new customers under an arrangement called the Supply Extension Scheme (SES).  Conditions apply and are outlined in the application form below.

Unmetered Electricity Supply Application Form

This form is required for unmetered electricity supply connections in public places, eg. traffic lights, illuminated street signs, local government owned street lights, telecommunication equipment, etc.
