New Electricity Connection Application
This form must be submitted to Horizon Power to establish a new customer electricity account. To obtain a new supply connection, an Electrical Contractor's Preliminary Notice is also required with this form.

Application for the Installation of CT Metering
This form is required to arrange for Current Transformer (CT) metering when there will be no increase in demand, eg replacement of existing switchboard or metering.

Automated Meter Reading (AMR) System Application
This form is to be submitted to Horizon Power prior to an AMR system being installed. It outlines the project details and enables Horizon Power to liaise with the AMR installer in relation to metering and communication issues.

Re-Inspection Fee Review
This form is to be completed and submitted to Horizon Power's inspectorate where the electrical contractor disagrees with the application of an Inspector's Order (I-type) and requires a review of the re-inspection fee.

Demolition Notice
This form is to be completed and submitted to Horizon Power's Connectin Office before a property is demolished. Customers must contact 1800 267 926 (residential) or
1800 737 036 (business) to finalise their account before the supply can be disconnected.
