Completed Supplier Request Form (obtainable from authorised Horizon Power employee or included in Supplier Accreditation pack).
Notification on company letterhead of Electronic Funds Transfer (EFT) information.
Completed and signed the Insurance Schedule for suppliers of services only (not goods). This needs to be authorised by your Insurance Broker.
Signed System Level Accreditation Acceptance Form.
